EarlyCSE had inconsistent behavior with regards to flag'd instructions:
- In some cases, it would pessimize if the available instruction had
different flags by not performing CSE.
- In other cases, it would miscompile if it replaced an instruction
which had no flags with an instruction which has flags.
Fix this by being more consistent with our flag handling by utilizing
andIRFlags.

FCmp behaves a lot like a floating-point binary operator in many ways,
and can benefit from fast-math information. Flags such as nsz and nnan
can affect if this fcmp (in combination with a select) can be treated
as a fminnum/fmaxnum operation.
This adds backwards-compatible bitcode support, IR parsing and writing,
LangRef changes and IRBuilder changes. I'll need to audit InstSimplify
and InstCombine in a followup to find places where flags should be
copied.

The raw non-instruction/constant form of this is still relying on being
able to access the pointee type from a pointer type - those will be
cleaned up later. For now, just focus on the cases where the pointee

This happened to be fairly easy to support backwards compatibility based
on the number of operands (old format had an even number, new format has
one more operand so an odd number).
test/Bitcode/old-aliases.ll already appears to test old gep operators
(if I remove the backwards compatibility in the BitcodeReader, this and
another test fail) so I'm not adding extra test coverage here.

m_ZExt might bind against a ConstantExpr instead of an Instruction.
Assuming this, using cast<Instruction>, results in InstCombine crashing.
Instead, introduce ZExtOperator to bridge both Instruction and
ConstantExpr ZExts.

"Setting" does not equal "copying". This bug has sat dormant for 2 reasons:
1. The unit test was not adequate.
2. Every current user of the "copyFastMathFlags" API is operating on a new instruction.
(ie, all existing fast-math flags are off). If you copy flags to an existing
instruction that has some flags on already, you will not necessarily turn them off
as expected.
I uncovered this bug while trying to implement a fix for PR20802.

MSVC on x64 requires that we create image relative symbol
references to refer to RTTI data. Seeing as how there is no way to
explicitly make reference to a given relocation type in LLVM IR, pattern
match expressions of the form &foo - &__ImageBase.

into their new header subdirectory: include/llvm/IR. This matches the
directory structure of lib, and begins to correct a long standing point
of file layout clutter in LLVM.
There are still more header files to move here, but I wanted to handle
them in separate commits to make tracking what files make sense at each
layer easier.
The only really questionable files here are the target intrinsic
tablegen files. But that's a battle I'd rather not fight today.
I've updated both CMake and Makefile build systems (I think, and my
tests think, but I may have missed something).
I've also re-sorted the includes throughout the project. I'll be
committing updates to Clang, DragonEgg, and Polly momentarily.
