Summary: This patch is an attempt at making `DenseMapIterator`s "fail-fast". Fail-fast iterators that have been invalidated due to insertion into the host `DenseMap` deterministically trip an assert (in debug mode) on access, instead of non-deterministically hitting memory corruption issues.
